Supporting elderly residents and their families

For more than 30 years Gunning Community Care has supported the aged in our community with a dedication and commitment to client-focussed care.


Support to stay at home for as long as possible
We want to support elderly residents to live independently in our community for as long as they are able.


Gunning Community Care works with aged people, including veterans, and their families to develop a suitable care plan depending on the individual health and support needs of each person and respects their rights.


We support carers and families with the care they provide and understand the additional challenges aged people and their families face in rural or isolated communities.

Aged services provided by Gunning Community Care

Our service comprises a Client Support Administration and a Community Nurse to monitor chronic health conditions, provide wound and palliative care, cancer support, medication management and pain assessments.

 

Clinics and home visits for treatment are available.

Our organisation offers full case management for the aged, including:

- complete care for residents living with dementia

- respite care

- social support

- domestic assistance,

- an equipment borrowing service

- a social group

- meal services

- transport to medical appointments

- palliative care

Gunning Community Care is an accredited Commonwealth Home Support Programme (CHSP) and Home Care Package (HCP) provider and delivers aged care support services under the new My Aged Care system. 

 

How do I arrange help?

When an aged person enters the care of our organisation, whether it’s for a short term or for ongoing care and support, they will be visited in their home by our Client Support Officer and one of our qualified carers.


This meeting will identify the needs and expectations of the aged person and their families, the types of care they require and the funding assistance they may be eligible to receive.


We also assist families with the difficult task of moving loved ones with dementia into care facilities and provide support to these families.
